Description :
The Hospice RN Case Manager is responsible for coordinating and managing the care of patients in a hospice setting, providing skilled nursing services and overseeing the implementation of care plans. This role requires strong clinical knowledge, excellent communication skills, and the ability to manage patient care autonomously while collaborating with an interdisciplinary healthcare team.
Primary Responsibilities and Duties :
- Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
- Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
- Coordinate and supervise the activities of hospice aides and other healthcare professionals involved in patient care.
- Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
- Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
- Collaborate with the interdisciplinary team to develop and implement patient care strategies that align with patients' wishes and goals.
- Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
- Provide bereavement support and resources to patients' families and caregivers as needed.
- Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
- Perform all other duties assigned by the Hospice Director or designated supervisor.
Requirements :
Current Registered Nurse (RN) license in the state of practice.1 year preferred hospice experienceMinimum of one year of experience as an RN in a hospice or related healthcare setting.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal skills.Ability to work independently and manage time effectively.Valid driver's license and reliable transportation for home visits.Current CPR certification.Knowledge of regulatory requirements and guidelines governing hospice care and documentation.Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting.Experience with electronic medical record systems and documentation preferred.Physical Requirements :
Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.Capable of prolonged standing, bending, squatting, and walking during home visits.Fine motor skills for documentation and administering treatments.Ability to respond swiftly and efficiently to emergency situations.Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor / manager.Job requires accepting criticism and dealing calmly and effectively with high-stress situations.The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.Schedule :
On-CallFull time Mon-Friday 9am - 5pm
