This is a Career Service Records Technician position working for the Lee County Health Department. This is a routine technical position involving independent judgment and initiative in the application of departmental operations and procedures in the Vital Statistics Office of a large county health department. This position will train and maintain appropriate knowledge, skills and abilities of the e-Vitals computer system. Incumbent will acquire and maintain a good rapport with customers, funeral homes, and the Medical Examiner's Office.
As a Deputy Registrar in a large county health department, incumbent will review death, fetal death, and birth certificates on a daily basis for completeness, accuracy, neatness, visual errors, corrections, use of white-out and the timely filing of the certificates. Employee signs, dates, alphabetized by date of death, numbers, logs and updates statistical report for errors and enters death certificates on hand on spreadsheet and investigates any discrepancies. Employee mails original certificates using the contracted shipping vendor, for permanent registration with the Bureau of Vital Statistics in Jacksonville. Employee reviews and issues Burial Transit Permits to funeral directors as necessary. Employee correlates white / pink copies of the Burial Transit Permits and file, following established procedures.
Issues certified copies of death certificates to funeral directors per their requests.
Acts as alternate cashier accepting in-house and mail-in applications requesting certifications of birth and death records. Reviews applications for accuracy, completeness, identification and eligibility. Identification proof must be documented in the e-Vitals System, copied and attached to each application. Enter demographic and accounting information into the same system. The employee will receive payment and issue receipt for each chargeable transaction. Compile all monies received, adding cash, checks and credit card payments for the deposit. The employee is responsible for completing the proper deposit forms of all monies collected and forwarding such monies to the Accounting Department.
Information Custodian Designee : This position has the responsibility of being the Alternate Information Custodian for the Vital Statistic information set and, in the absence of the Information Custodian will perform related duties as defined in DOHP 50-10.
Acts as backup to print and reconcile the CDR Hospital Birth Log from e-Vitals with the faxed copies of Birth Logs from three (3) major Lee County hospitals at the beginning of each week, checking for any discrepancies. Employee reviews and processes online orders via VitalChek. Performs other duties as assigned.
