Physical Therapist - Home Health - (PT - HH)

Pay Rate : $1984.00 - $2063.00

Travel Physical Therapist (PT) jobs in Hamden, CA for Home Health offer a 13-week contract with 8-hour day shifts. In this role, you will provide patient-centered care by evaluating, planning, and implementing treatment programs to improve mobility and enhance quality of life in patients’ homes. You will focus on helping patients regain independence, educate families on therapy plans, and document progress accurately.
