Position Overview
This role is dedicated to delivering clinical and palliative care services within hospital or homecare environments. The practitioner supports the assessment, development, implementation, monitoring, and evaluation of clinical programs and policies. They actively contribute to quality improvement initiatives and collaborate closely with physicians and healthcare teams to optimize patient outcomes and resource management.
Key Responsibilities
- Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
- Provide clinical services in hospital settings, including patient health assessments and examinations.
- Monitor patient outcomes, identify barriers to care, and collaborate with interdisciplinary teams to address challenges.
- Educate physicians, inpatient staff, and the community on hospice and end-of-life care services.
- Participate actively in performance improvement programs.
- Develop and evaluate clinical policies, procedures, forms, and monitoring tools.
- Coordinate efforts to maximize clinical effectiveness and financial efficiency.
- Work alongside medical directors, physicians, and healthcare professionals to deliver comprehensive clinical care.
- Engage with infection control departments on reporting, screening, and prevention programs related to infectious diseases.
- Prepare and present monthly reports and statistics to management; attend relevant meetings and workshops.
Qualifications
Minimum of 2 years experience as an adult Nurse Practitioner preferred; candidates with less NP experience but at least 5 years of RN acute care experience will be considered.Completion of all Nurse Practitioner clinical requirements.Proficient computer skills.Excellent communication and customer service abilities.Bilingual in English and Spanish is strongly preferred.Education
Graduate of an accredited nursing school.Master of Science in Nursing (MSN) degree required.Licenses and Certifications
Current Registered Nurse (RN) license for the state of California.Current Nurse Practitioner license for the state of California.Basic Life Support (BLS) certification from the American Heart Association required upon hire.All candidates must successfully complete a physical evaluation, drug screening, and background checks prior to employment. Compliance with regional COVID-19 vaccination mandates is required.
