Nurse Practitioner / Physician Assistant – House Call Primary Care (Pensacola or Milton)

Full-Time | $100,000 – $130,000 annually | Mission-Driven Team

We care primarily for chronically ill, homebound older adults who thrive when they receive attentive, personalized medical care where they feel safest. If you feel called to serve this population with excellence, we would love to meet you.

What You'll Do

• Provide comprehensive primary care to adult and geriatric patients in their homes.
• Average 8 visits per day (or as many as you can comfortably and safely care for).
• Manage chronic conditions, perform routine assessments, and coordinate patient care.
• Collaborate with our supportive clinical and administrative team.
• Document visits electronically and maintain high clinical standards.
