Summary The Hampton VA Medical Center is a 1-C - teaching facility serving Eastern Virginia and Northeastern North Carolina. The main campus is located in Hampton, VA with 4 community-based outpatient clinics in Chesapeake, Portsmouth, and Virginia Beach, VA; and Elizabeth City, NC. Hampton VAMC provides comprehensive health care which includes primary care, acute inpatient, acute psychiatric, chronic spinal cord injury, an 169-bed domiciliary, and long term Community Living Center / hospice care unit.
Duties Duties include but not limited to : Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
Trains primary care teams on group visits as well
Develops and implements an individualized plan to achieve optimal population health and disease management
Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
Leads Mailed FIT Program
Orders and interprets labs, tests, and diagnostic studies
Prescribes pharmacologic and non-pharmacologic interventions
Initiates referrals and consults to specialty services
Provides education to patients and their families
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

Pay : Competitive salary; regular salary increases; potential for performance awards
