Nurse Practitioner - Family physician employment in California : Great opportunity in the Santa Clara / San Jose area of California for a Nurse Practitioner to perform annual wellness exams working for a private group.

• Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
• 1+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
• Perform an annual wellness exam for senior Medicare patients in clinic
• Collaborate with Primary Care team to ensure completion of holistic patient care
• Combination of home health and telehealth - 30 mile radius
• CA license and DEA required
• Mon-Fri 8a-5p
• 8-10 patients per day
