Our client is revolutionizing healthcare delivery with innovative solutions and on-site care. They need a Bilingual Case Worker to join their team.
Responsibilities :
- Conduct case work duties, meet with residents, and prepare evaluations.
- Inform residents about facility rules and service plans.
- Maintain confidential records and handle referrals to agencies.
- Assist residents with outside services and arrange transportation.
- Attend meetings and document progress.
