Pay Range : $33 - $59 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Deliver exceptional patient care : Perform assessments, develop and execute care plans, and monitor patient progress with attention to detail and empathy.
• Coordinate care : Serve as the bridge between patients, families, and multidisciplinary care teams to ensure clear communication and continuity.
• Advocate for patients : Educate and empower patients and families about treatment options and health maintenance.
• Collaborate for excellence : Work alongside care teams to improve outcomes, safety, and satisfaction.
