Job Overview
Under the direction of the Health Education & Wellness (HEW) Supervisor, the Wellness Program Coordinator is responsible for the overall implementation and success of various wellness programs, targeting pediatric obesity risk-reduction, pre-diabetes, diabetes prevention / management and more at the site level. Specific responsibilities to include : monitoring participant / patient screening process, enrolling program participants, updating / managing patient’s files throughout all program phases, overseeing scheduling of sessions, tracking and monitoring patient’s progress, and ensuring that data collection and reporting processes are accurate and valid. He / she will ensure all wellness programs are following proper protocol adherence at all times.
S / he will function as an AltaMed ambassador, welcoming and attentive to the needs of all patients and helping to create a positive work environment and patient experience by managing expectations and demonstrating compassion and respect. Program Coordinator is responsible for relationship building with patients; specifically utilizing Motivational Interviewing skills to foster positive environment for participant.
Minimum Requirements
1. High School diploma or GED required; Bachelor’s degree preferred.
2. English / Spanish required due to population served.
3. Current Valid BLS Certification
4. Proficiency in MS Office applications, including MS Work, MS Excel and Outlook required.
5. Experience working with community-based programs a minimum of 4 years required.

$23.00 - $27.65 hourly
