We are seeking a compassionate, dedicated, and team-oriented Nurse Practitioner OR Physician Assistant to serve as a Primary Care Provider (PCP) at our PACE location. The Advanced Practice Provider will deliver participant-centered care within an interdisciplinary team, directly contributing to the health outcomes and quality of life of our participants. This role requires a combination of direct patient care, team support, and clinical administrative responsibilities. Schedule : Monday-Friday 8am-5pm.

Responsibilities

Direct Patient Care :

• Provide comprehensive primary care to PACE participants, expertly managing acute and chronic health conditions.
• Perform initial and ongoing comprehensive physical examinations and thorough health assessments.
• Order, administer, and accurately interpret diagnostic tests such as laboratory tests, electrocardiograms, and imaging studies.
• Conduct therapeutic procedures, including injections, immunizations, wound care, and managing infections.
• Offer preventive care services, medication management, and coordinate necessary specialty care services.
• Conduct home visits to evaluate participant safety, functionality, and overall well-being, ensuring comfort and dignity in their living environment.
• Educate and counsel participants and families about prescribed therapies, health maintenance strategies, and lifestyle modifications.
• Identify and address psychosocial, emotional, and environmental factors affecting participant health.

Pay range : $135,400 - $211,200 annually, depending on experience.

Compensation Disclosure : The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
