job_description.job_card.job_descriptionConduct routine and comprehensive ESKD rounds under the guidance of nephrologists. Manage holistic patient care, including : Depression and mental health support Hospitalization risk reduction Transition of care and co-morbid condition oversight Provide patient education on ESKD and kidney disease. Perform annual disease management assessments. Collaborate with nephrologists, dialysis center teams, and other healthcare providers. Coordinate care with PCPs and specialists as clinically indicated. Support discharge planning and continuity of care. Perform physical examinations to evaluate health and diagnose illnesses. Order and interpret diagnostic tests (e.g., x-rays, lab tests). Develop and adjust treatment plans; prescribe medications as needed. Document patient information and maintain electronic medical records. Educate patients and families on treatment plans, preventive care, and healthy behaviors. Facilitate consultations and referrals with other healthcare providers.
We are seeking a dedicated healthcare professional to provide exceptional care and support for patients with End-Stage Kidney Disease (ESKD). This role focuses on holistic patient management, education, and collaboration with multidisciplinary teams to ensure the highest quality of care.
Location : San Jose, CA
Hours : M-F 40 hours / week
Requirements :
- Current specialty certifications and license to practice as an Advanced Practitioner in the state Interested
- Enrolled in CMS (Medicare) of the state
- Firm understanding of the protocols, policies, and operations of a physician's practice
- Minimum of 2 years of NP experience
- Experience working in Nephrology
- Comfortable traveling between facilities and working independently
