Lincoln Community Health Center, Inc., a federally qualified community health center located in Durham, NC is recruiting for a Population Health Representative . Durham is known as the Bull City, the City of Medicine, the Tastiest Town in the South, and the Startup Capital of the South. It's a two-hour drive from the beach, a three-hour drive from the Blue Ridge Mountains, and is North Carolina's fourth largest city by population. Along the East Coast, Durham is located conveniently along I-85. OCCUPATIONAL SUMMARY

The Population Health Representative serves in an administrative role within the Department of Population Health, collaborating with all departments to strengthen the patient-clinician relationship by supporting the planning and coordination of care across the healthcare spectrum. This role is primarily telephone-based, involving making and receiving phone calls, conducting targeted outreach to engage patients, managing the hospital follow-up line, and promoting access to care. The comprehensive care provided by the Population Health Representative aims to promote better health, longer lives, higher patient satisfaction, and less expensive care.

MINIMUM QUALIFICATIONS

• High School diploma or equivalent.
• A minimum of one-year experience in a healthcare setting.
• Knowledge of medical terminology.
• Experience working for a non-profit community health clinic and / or social service organization.
• Bi-lingual required
