Job Title & Role Description

The Nurse Practitioner (NP) is responsible for providing high-quality, patient-centered, comprehensive primary care . Using a proprietary facilitated virtual care platform, the NP delivers care that improves outcomes and reduces costs for patients with complex diagnoses and multiple chronic conditions.

This role is primarily virtual , with periodic or as-needed in-person visits for care transitions, annual wellness visits, risk adjustments, urgent visits, and other primary care needs. The NP works closely with a multidisciplinary team to provide whole-person care directly in patients homes.

Key Responsibilities

• Triage, diagnose, and treat patients in person or via the virtual care platform
• Serve as a day-to-day clinical leader , providing decision support and guiding the multidisciplinary team under the direction of the Medical Director
• Conduct primary care visits , urgent visits, and annual wellness exams , ensuring all appropriate screenings and quality measures are addressed
• Refer patients to behavioral health and specialty services as appropriate
• Coordinate care transitions to prevent hospital readmissions by collaborating with PCPs, hospitalists, and SNF providers
• Participate in the on-call provider rotation for the 24 / 7 patient hotline, including after-hours and weekends as scheduled
• Foster a collaborative working environment by contributing to daily huddles, team meetings, and patient care discussions
• Monitor and respond to remote patient data collected via monitoring devices
• Perform other duties as assigned
