Behavior Health Specialist

POPULATION SERVED : Demonstrates the knowledge and skills necessary to provide care

appropriate to the age of the consumers served in the department :

ü Infant & Early Childhood (birth – 5 years)

ü Child (6 – 11 years)

ü Adolescent (12 – 17 years)

Essential Functions :

1. Conducts observations of concerning behaviors in home, in school and in the community to

identify triggers, drivers, and reinforcers of behaviors.

2. Develops and utilizes evidence based and best practice in skill building sessions to support

development of healthy and safe skills to mitigate concerning behaviors.

3. Provides in-home, in school and community-based coaching / direct support services for at-

risk youth and families.

4. Teaches a variety of skills (living, social, self-care, emotional regulation, executive

functioning, household management, etc.) to children, adolescents, parents, school officials,

teachers, and caregivers.

5. Provides caregiver sessions consistently to offer support, guidance and training to help

improve family functioning.

6. Identifies youth and family strengths and needs to enhance the Child and Family Team

(CFT) planning process and may facilitate and participate in CFT meetings.

7. Identifies, connects, and integrates families in to a variety of community resources and

outlets that build off individual and family strengths.

8. Works collaboratively with all Touchstone Behavioral Health departments and employees,

members, youth, families, and schools being served.

9. Communicates to Supervisor all updates, progress, or barriers being encountered during

daily practice.

10. Completes all required clinical documentation and reports within timeliness guidelines of agency. Maintains Quality Assurance Procedures and Quality documentation which includes updating consumer documentation as appropriate to maintain an accurate Electronic Health Record and compliance in accordance with regulatory and agency expectations.

11. Performs all duties in accordance with Child and Family Team process, Arizona model and in accordance and compliance with payors.

12. May conduct Child and Family Team meetings and assessments as needed for Touchstone members.

13. Performs all other duties as assigned.
