Description

The Nurse Practitioner Diffs examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention diagnosis treatment and recovery. Relies on education experience professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician as appropriate. May lead and direct the work of others within scope of practice.

Responsibilities

1. Patient Care : This will have been satisfactorily performed when

• Patient assessment and teaching and learning needs are accurately documented detailed nursing history is obtained and patients at physiologic and or psychological risk are identified as evidenced by physical assessments of patients that identify priority care needs.
• Diagnosis is appropriately formulated with pertinent therapeutic and diagnostic procedures ordered at all times as evidenced by necessary procedures that are performed to implement plan of care based on care standards and or protocols.
• Treatment plan is implemented with consults and referrals with interdisciplinary team regarding patient assessment findings conducted as appropriate at all times.
• Plan of care is reassessed revised and modified based on physiological and psychosocial findings as evidenced by documention in the treatment plan. Patient plan of care is reviewed with interdisciplinary team at all times.
• Tests and consults are ordered as required by plan of care reviews test results 100% of the time. Plan of care is discussed with patient and family at all times as evidenced by documentation.
• Education is provided to patients based upon identified risk factors as evidenced by patient education programs that are developed revised and evaluated based on the effectiveness of learning.

Advanced Practice License from Louisiana State Board of Nursing and Certification as Nurse Practitioner by ANCC or AANP and Current Louisiana state RN license and Current BLS Certfication (within 6 months of hire)
