The ideal candidate will possess the following qualifications :

• High school diploma or GED required.
• Associate's degree with focus in psychology, social work, or public health administration preferred .
• Certification as a Community Health Worker through the PA Certification Board required, or must be obtained within one year of hire.
• Lived experience within the population to be served, which may include aspects such as geography, physical or behavioral health, race, immigration status, SOGIE identification, etc required.
• Must have valid driver's license and access to a vehicle.
• Knowledge of chronic health conditions preferred.

Position Details

Community Health Workers (CHWs) possess a deep understanding of the community they serve and build trusting relationships with individuals from diverse backgrounds to connect them with resources and care. Their effectiveness stems from their ability to reach underserved and hard-to-reach populations, providing patient support and coaching to achieve healthcare goals by addressing Social Determinants of Health (SDOH) needs. This role involves both face-to-face and telephonic interactions with individuals, families, and the community.
