How You Support Our Mission :

The Pure Healthcare Care Manager will develop comprehensive person-centered care plans for every member, with a focus on assuring timely and appropriate access to care and the skillful coordination of broad array of medical and social services. This position affords the opportunity to work with individuals, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care from acute care across the continuum. The Care Manager is the key to ensuring a collaborative and proactive approach to support the individual and caregiver in the management chronic medical conditions.

Job Summary / Purpose : The Pure Healthcare Care Manager will develop comprehensive, person-centered care plans for every member, with a focus on ensuring timely and appropriate access to care and the skillful coordination of a broad array of medical and social services. This position offers the opportunity to work with patients, providers, and multidisciplinary team members to assess, facilitate, plan, and coordinate the integrated delivery of care across the continuum, from acute care to the broader healthcare system. The Care Manager is the key to ensuring a collaborative and proactive approach to support the patient and caregiver in the management of chronic medical conditions.
