Key Responsibilities of LMFT

• Collaborate with medical staff to coordinate patient care manage referrals and support treatment planning and discharge processes.
• Assess patients and develop individualized treatment plans in collaboration with the care team.
• Provide education to patients families and staff and ensure timely updates to care plans and coordination of care transfers.
• Provide psychological assessments therapy and resource referrals for patients and families.
• Develop and implement appropriate psychological interventions to support patient care.
• Launch and help develop the Clinical Social Work Intern Program

$100000- $110000 annually based on experience
