Rehabilitation Manager – Managed Long-Term Care (MLTC)

Location : Kew Gardens, NY

Hybrid schedule : 3 days in-office, 2 days remote, with home visits as needed. Coverage area includes Queens, Long Island, and North Brooklyn.

Overview :

We are seeking a Rehabilitation Manager to support the clinical oversight and delivery of rehabilitation services within Managed Long-Term Care (MLTC), MAP, and ESNP programs. This role focuses on ensuring members receive appropriate care and equipment to support independence and quality of life at home.

Responsibilities :

• Review rehabilitation documentation from external providers to ensure services are clinically appropriate.
• Evaluate and process requests for Durable Medical Equipment (DME) for home use.
• Provide education and guidance to nursing leadership regarding rehabilitation services and care planning.
• Perform home and field assessments as needed (including evaluation for complex DME and home safety).
• Maintain compliance with New York State Department of Health regulations.
