As a Community Health Worker, you will serve as a vital liaison between health providers, social service organizations, and the community. You will help patients overcome barriers to care, provide education on reducing health risks, and connect them with services that promote long-term wellness. Our ideal candidate is knowledgeable about community health resources, skilled in patient engagement, and committed to reducing health disparities.
