About the Role :
As a Patient Navigator LPN, you will play a crucial role in ensuring that patients receive the best possible care. You will be responsible for guiding patients through the healthcare system, providing them with the support and resources they need to navigate complex medical procedures and treatments. Your primary goal will be to ensure that patients receive timely and effective care, and that their needs are met throughout the entire healthcare process. You will work closely with physicians, nurses, and other healthcare professionals to coordinate care and ensure that patients receive the highest quality of care possible.
Minimum Qualifications :
- Active LPN license in the state of employment.
- Minimum of 2 years of experience in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
Preferred Qualifications :
Experience working as a patient navigator or care coordinator.Experience working with electronic medical records (EMRs).Bilingual in English and Spanish.Experience working with underserved populations.Responsibilities :
Conducts assessments, develop care plans and follow-ups of clients to ensure ongoing support.Address social determinants of health, identify barriers to care, manage high-risk pregnancies.Provide education on high-risk pregnancies, decrease barriers for appointment attendance, and medication adherence within their scope of services.Refer clients to services and programs within the HHC and outside of the agency.Advocate for clients, helping them overcome barriers to care, such as language, transportation, and financial constraints.Coordinate care with healthcare providers, including OB-GYNs, hospitals, primary care physicians, and other specialists.Conduct face-to-face visits, virtual consultations and telephonic outreach as needed.Utilize client data management systems to maintain comprehensive patient records, documentation of assessment, care plans, interventions and track outcomes.Participate in outreach initiatives and community events to promote health education and improve access to care for underserved communities.Performs related work as required.Skills :
As a Patient Navigator LPN, you will use your strong communication and interpersonal skills to build relationships with patients and their families, and to collaborate effectively with healthcare professionals. You will also use your organizational and time management skills to ensure that patients receive timely and effective care. In addition, you will use your knowledge of medical terminology and procedures to maintain accurate patient records and educate patients about their medical conditions and treatment options. Finally, your ability to work independently and as part of a team will be essential in ensuring that patients receive the highest quality of care possible.
