Women's Health Concierge

Trinity Health Of New Englanda is looking for a Women's Health Concierge to join our Comprehensive Women's Health Center. The Women's Health Concierge is an administrative support position who provides patient care coordination by serving as a liaison between the patient and the practitioners and services she may require. The Women's Health Concierge plays a critical role in the patient experiences and acts as a patient advocate, provides patient and family education, and coordinates all aspects of care to assure the provision of seamless comprehensive services.

What will you do :

• Become an integral part of the organizations commitment to provide holistic patient centered care for women.
• Optimizes patient services and collaborates with the team to create a patient centered environment.
• Provide expertise in the operations and systems of the health center to promote and maintain health and wellbeing in women.
• Monitors and manages the patient referrals.
• Schedules appointments for procedures, surgeries and diagnostic testing.
• Answers phones for the Women's Health Center.
