LPN Case Manager

A company is looking for a Case Manager - LPN / LVN.

Key Responsibilities

Manage cases telephonically on a short- and long-term basis

Administer assessments to evaluate patients' healthcare needs and collaborate on individualized plans

Monitor treatment plans and establish relationships with patients and healthcare providers

Required Qualifications

Graduation from an accredited LVN / LPN program and a current LVN / LPN license

Prior experience in case management or wellness program coordination

Experience in medical, surgical, or acute care settings

Understanding of case management and health program coordination

Proficiency in Microsoft Office Suite and knowledge of medical claims coding preferred
