How youll contribute

• Facilitates the patients continued recovery after discharge. Provides aftercare plan coordination for patients who need ongoing medical / psychosocial care following collaboration with other members of the healthcare team assesses medical needs of the patient and offers suggestions regarding follow-up care options.
