Hours per Shift : Monday - Friday 8 : 00 AM - 4 : 30 PM; weekends as needed
Experience : Minimum of one year in Long Term Care / Home Care setting as a Physical Therapist. Recent Home Care experience within the last year.
License : Possession of a current Client License in Physical Therapy.
Certifications : BLS (AHA ONLY)

Description :
Provides case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home setting.
Assists the physician in evaluating the level of function and helps develop the plan of care, revising it as necessary.
Prepares clinical and progress notes.
Advises and consults with the family and other personnel.
Participates in in-service programs.
Participates and ensures that services are consistent with the organization's vision, mission, values, and strategic goals.
Promotes effective working relationships and works effectively as part of the team to meet objectives.
Utilizes resources, supplies, and time efficiently to contain costs and increase service value.
Promotes positive, supportive, respectful communication to all external sources, patients, families, and staff.
Displays behaviors that support the overall organization.
Direct Patient Care Duties :
Develops and revises treatment and discharge plans in conjunction with the patient, family, physician, and care delivery team.
Provides an environment that respects patients' privacy, confidentiality, and property.
Initiates appropriate rehabilitative programs.
Provides supervision of PTA to ensure patient health, safety, and compliance with the plan of care.
Consults with and educates the patient, family, and other team members regarding disease processes, self-care techniques, and preventive strategies.
Implements home therapy programs including gait training, transfers, activities of daily living, home modification, and use of equipment.
Periodically evaluates patient progress and makes necessary adjustments to the physical therapy program.
Instructs para-professional staff in body mechanics, use of equipment, and home exercise programs.
Coordinates, evaluates, and supervises the quality of patient care services provided.
Interacts with patients and others in a manner conducive to continued positive relationships.
Displays patience, courtesy, and tact.
Supports collaborative, customer-focused planning and case management designed to meet individual health and service needs.
Participates in regularly scheduled patient conferences for the discussion of patient care and to ensure a comprehensive coordinated plan of treatment.
Assists in the orientation of new rehabilitation personnel.
Completes all appropriate clinical records needed for compliance with state and federal legislation.
Refers to appropriate community agencies / resources.
Maintains current knowledge of available services.
Demonstrates flexibility, enthusiasm, and willingness to cooperate while working with customers and others.
Ensures problem resolution at the point of contact.
Makes constructive recommendations regarding ways to improve the quality of patient care and external customer service.
Sets an example of integrity and ensures patient needs are met through the care delivery process.

Participates in the attainment of operational and financial objectives.
