Outpatient Physical Therapist

CLIENT : Banner

SETTING : Outpatient

START DATE : Late February

HOURS : 40 hours scheduled. In case of low census or cancellations, only actual hours worked will be billed / paid if the provider leaves.

ASSIGNMENT LENGTH : 13+ weeks (longer preferred)

EXPERIENCE : Preferred 1+ year outpatient experience

STAFFING : Collaborate with other skilled PTs

EMR SYSTEM : Therapy Source / Net Health

CASELOAD : Expected 10 visits per day during an 8-hour shift

SCHEDULE : Monday to Friday

ADDITIONAL DETAILS : This role includes working in a dynamic clinic environment.

COVID VACCINE : Required

BILL RATE : $68-$75 based on experience (5% fee applies)

NOTES :

• 1st day is unbillable orientation.
• Documentation must be completed at the point of service for all staff.
• 14-day notice required for termination. Failure to provide notice may result in billing for a maximum of 72 hours minus hours worked post-notice.
• Any requested time off not on the approved list must be approved by management 30 days in advance.
• In case of low census or cancellations, providers are expected to inquire about ways to generate hours at the facility.
• In the event of clinic closure due to inclement weather, only actual hours worked will be billed.
• Overtime must be preapproved in writing; paid at the same rate as the regular bill rate.
• Select recognizes holidays to be billed at 1.3 times the normal hourly rate : Christmas Day, New Year's Day, Memorial Day, Fourth of July, Labor Day, and Thanksgiving.
• Buyout options : $0 after 13 weeks; $5K before 13 weeks.
