job_description.job_card.job_descriptionHome health setting covering East Portland and surrounding areas Day shift schedule with 8-hour shifts, 8 : 00-16 : 30, five days per week Patient ratio is 1 : 1 with up to 6 visits per day and 30 productivity points as standard No call, weekends, or floating to like units required Travel is necessary, with occasional drive times up to 60 miles or 60-90 minutes per day Epic is used for documentation through Remote Client and Rover app; Oasis documentation experience preferred Mileage reimbursement provided at IRS standard rate Responsibilities include delivering physical therapy services to home health clients with a focus on productivity and quality outcomes All travelers must be willing to float between all facilities within the state if required Business casual attire is required; scrubs permitted but not mandatory Free parking provided First-time travelers are accepted for this role Locals within 50-75 miles are accepted at the same rate Returning employees must have a break of at least 3 months and reside more than 50 miles from the facility Minimal to no RTO is allowed; any requests for time off must be confirmed with hiring manager prior to submittal Pre-assignment modules are required and must be completed prior to start Minimum 1 year of recent skilled home health experience required Current BCLS certification required OASIS documentation experience required or must be clearly indicated if lacking Epic EMR experience recommended Ability to provide two recent supervisory references within the specialty Primary series COVID vaccination plus one booster required Valid driver's license and reliable transportation required License required if applicable High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
