HOW YOU'LL MAKE AN IMPACT

Your Role :

Provides skilled nursing care on an intermittent basis to patients in their home in accordance with physician's orders.

Coordinates with other medical personnel and community resources to meet the needs of the homebound patient.

Maintains all aspects of the clinical record.

Reports To :

RN / Nurse Manager (Job Code (JC) : 030303)

Supervises :

LPN

Home Health Aide

WHAT YOU'LL NEED TO APPLY

Required Education :

Associate degree in Nursing

Preferred Education :

Bachelor's Degree in Nursing

Required Experience :

One (1) year of prior Medical Surgical Nursing experience.

Preferred Experience :

Two (2) years of Home Health Care experience.

Required Certification / License / Registry :

RN Licensure in the State of Florida

Must have a current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an accredited instructor from the American Heart Association (AHA) and / or other specialized life support requirements designated by work area.

Job requires a valid state driver license.

Required to provide a seven (7)-year driving record from the Department of Motor Vehicles (DMV) prior to employment. Will have received no more than two (2) minor violations in the past three (3) years and zero (0) major violations in the past five (5) years.

Required to be at least twenty-one (21) years of age and with a minimum of two (2) years of licensed driving experience.

NOTE : Use of personal vehicle(s) required. Vehicle owner must maintain adequate insurance coverage for personal vehicle(s) used in conjunction with duties. Tallahassee Memorial HealthCare (TMH) will reimburse the business use of personal vehicle(s) in accordance with procedures defined by the Internal Revenue Service (IRS).
