Practice Overview

• You'll lead a small Memphis-based team of 2 APPs while managing a personal patient panel
• Home-based care model with no clinic, hospital, or facility-based responsibilities
• Manage chronic conditions for approximately 140 homebound patients
• Serve as supervising physician, mentor APPs, lead care planning, and conduct ride-alongs
