Community Living Center Nurse Assistant

The Community Living Center is a mix of short stay rehabilitation, long term and palliative care residents. He / she is expected to work closely with the licensed staff and interdisciplinary team to deliver comprehensive, efficient, coordinated care to residents with complex medical conditions. He / she approaches each resident's care from a holistic perspective focusing on the resident's physical, psychological, social, and spiritual well-being.

Responsibilities include :

• Utilizing glucose monitoring machine to receive glucose readings for veterans and referring deviations to appropriate health care staff.
• Providing direct care for complex veterans.
• Making observations of subtle changes in resident's condition and taking action accordingly.
• Documenting care given, observations, and responses on appropriate tools.
• Performing nursing procedures within the scope of practice.
• Working on small group activities with veterans / residents.
• Recognizing residents' autonomy and promoting independence.
• Participating in treatment planning meetings.
• Utilizing equipment to promote veteran self-care.
• Maintaining a clean, safe environment for the patient / resident.
• Serving as the unit safe patient handling liaison.
• Acting as a preceptor and mentor to other NAs.
• Managing distributed patient behavior and intervening appropriately.
