The Director of Case Management leads and shapes the Case Management and Social Work services. As the Director of Case Management youll guide discharge planning Care Coordination and access management to ensure patients receive seamless support throughout the Continuum of Care. Additionally the Director oversees pre-certification for post-acute services optimize resource use and promote ICD-10 awareness. As a strategic leader passionate about improving patient transitions and building strong care connections this role offers the chance to make a meaningful impact system-wide.

Qualifications :

• Registered Nurse
• Bachelors Degree required
• Master Degree preferred
• Director of Case Management experience in an Acute Hospital Setting required
• ACM or CCM preferred
