Key Responsibilities

Engage and assess individuals with mental health conditions and unmet social service needs

Navigate individuals to appropriate behavioral health and primary care services

Collaborate with the clinical team and manage the care navigation process

Required Qualifications

Hold a Registered Nurse license or Clinical Social Work compact license

3+ years of experience in a health plan or at-risk provider setting

2+ years of experience in care coordination or case management

2+ years of contact center experience focused on patient outreach

Demonstrated experience working with individuals facing complex chronic needs
