job_description.job_card.job_descriptionImplement the individual’s Person-Centered Support Plan Assist with Daily Living Skills including but not limited to, meal preparation, feeding, personal hygiene / grooming, bathing, as necessary. Administer Medication(s) as necessary Complete required daily / monthly documentation Provide a safe and nurturing home to an adult with intellectual and / or developmental disabilities Must have a HS Diploma Must have a home that meets the requirements for the program. Home will be inspected by Agency Staff to determine suitability. Current CPR / First Aid Certification Medication Administration Certification (can be obtained after hiring) Active homeowner’s / renter’s insurance Valid Driver’s License and automobile insurance Must have reliable transportation and the ability to transport the client as necessary Experience working with adults with Intellectual / Developmental Disabilities $50 / day - $195 / day based on the level of the client.

About the Family Model Program
The Family Model Program is an opportunity for you to provide care to an adult with an intellectual and / or developmental disability in your home. Similar to the Foster Care model for Children, the Family Model Program offers the people we support an opportunity to live in a family home and receive the support they need to be as independent as they desire.
