job_description.job_card.job_descriptionThe Clinical Case Manager is responsible for planning, organizing, and coordinating swing bed admissions and reviewing emergency department admissions and discharges. Responsibilities include providing administrative and clerical services, input into process improvement, coordinating with the referral specialist and managing, obtaining, and sharing medical records. The Clinical Case Manager works under general supervision of the Director of Nursing. Essential Functions include financial assessment, providing information in response to queries, case management, social services, utilization review, and discharge planning. Coordination of swing admissions and verification of admission requirements, review of ED admissions / discharges, development and maintenance of relationships with specialty offices, participation in interdisciplinary team and UR meetings, review of patient charts for counseling requests, assessment of needs for intervention and / or community resources, education of patients and families about disease processes and treatment, connecting and advising ancillary departments of patient care plan changes. Three to five years of experience in clinical nursing. One to two years' experience in case management or related field preferred. Current Licensure to practice in the state of California. Bachelor's degree in nursing highly preferred. BLS required, ACLS preferred. Must be available at least two Saturdays a month depending on admissions. Specialty : Management Housing is a studio apartment offered at a rate. Boat Tickets are provided at the ferry ticket counter. No personal automobiles allowed on the Island.
