Job Description :

We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our home health care team in providing exceptional patient care in the Northwest side of town. The ideal candidates will have strong clinical skills, excellent communication abilities, and a passion for enhancing the quality of life for patients in their homes.

Key Responsibilities :

• Conduct home health visits to assess and monitor patients’ health status, including vital signs and medical history.
• Administer medications and treatments as prescribed by physicians.
• Assist patients with daily living activities, including bathing, dressing, and mobility, to promote independence.
• Collaborate with registered nurses and other healthcare professionals to develop and implement individualized care plans.
• Educate patients and their families on health management and medication adherence.
• Document patient care activities accurately and timely in electronic health records.
• Provide emotional support and encouragement to patients and their families.
• Participate in team meetings and contribute to continuous quality improvement initiatives.
