The RN or LPN Care Manager requires a strong foundation in healthcare management, proven leadership abilities, and a deep commitment to enhancing patient outcomes. The Lead Care Manager will be instrumental in guiding care management initiatives, supporting targeted interventions to reduce hospitalizations and emergency visits, and collaborating closely with clinical teams to optimize operations and ensure the delivery of high-quality care.
