We are seeking a dedicated and skilled Advanced Practitioner, either a Family Nurse Practitioner (FNP) or Physician Assistant (PA), to join our hospital-based practice. This position offers a Monday to Friday schedule with no call responsibilities, providing an excellent work-life balance. As an Advanced Practitioner, you will play a crucial role in delivering comprehensive healthcare services to our patients.

Responsibilities

• Patient Assessment

Conduct thorough assessments of patients, including medical history, physical examinations, and diagnostic tests to formulate accurate diagnoses.

Develop and implement individualized treatment plans, including medication management, preventive care, and health education, in collaboration with the healthcare team.

Manage chronic conditions by monitoring patients' progress, adjusting treatment plans, and promoting lifestyle modifications.

Provide immediate care for patients with acute illnesses or injuries, ensuring timely interventions and referrals when necessary.

Educate patients and their families on healthcare maintenance, disease prevention, and self-care strategies.

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.

Maintain accurate and up-to-date electronic health records (EHR) for all patient encounters.

Participate in quality improvement initiatives to enhance patient care outcomes.

Adhere to all regulatory and legal requirements, including licensure, certification, and hospital policies.

Stay current with advancements in healthcare through continuing education and professional development.
