Family Practice / Primary Care Nurse Practitioner

Outstanding primary care group in the Northern California area is seeking an experienced nurse practitioner to join their growing team!

This position is a mainly clinic-based role, with a small portion of in-home duties within the local community. You will deliver high-touch care to geriatric patients with the goal of preventing hospital readmissions, as well as managing chronic conditions and coordinating care across specialties.

Ideal candidates will have experience working as a nurse practitioner in primary care, family medicine, internal medicine, gerontology, or another relevant field.

Description :

• Full-time permanent position - Primary Care
• Monday-Friday schedule
• Focus on geriatric high-risk patients
• See patients mostly in clinic (about 75%) and some in-home visits (about 25%)
• No telehealth
• Work alongside a robust support team of medical assistants, MD advisor, and clinical leadership
• Deliver high-touch care to geriatric patients with complex needs
• Prevent hospital readmissions and reduce avoidable utilization
• Conduct AWVs, risk assessments, medication management
• Manage chronic conditions and coordinate care across specialties
• Participate in IDT meetings and quality initiatives
