Required Qualifications : Bachelor's Degree - In PHARMACY from an accredited School of Pharmacy or, Ph.D. - In PHARMACY from an accredited School of Pharmacy. Coursework / Training - Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent educ / experience. Oregon Pharmacist License upon hire. Within 2 years of hire : Board Certified by the Board of Pharmacy Specialties specific to area of practice. Pharmacy preceptor license in applicable state as applicable upon hire. 2 years - TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
