Your Role on the Team

Position Summary :

The Transplant Coordinator provides comprehensive management and coordination of care for Kidney and Pancreas Transplant patients. The level and intensity of care coordination is situational and based on patient / program need. The areas of transplantation which this position covers may include : pre-transplant evaluation, living donor, waitlist management, and post-transplant care. Transplant coordinators at UCI are required to provide 24 / 7, 365 day / year on-call coverage for patient care calls, organ offer management, and the coordination of transplant surgeries. Transplant coordinators serve as expert resources for patients, families, and the community, as well as other providers. Must possess advanced knowledge of, and adhere to, CMS / UNOS regulations and allocation policies. Will participate in the department quality assurance program. Transplant certification within three years of hire is strongly encouraged for this role.

THIS POSITION WILL BE FILLED AS EITHER A TRANSPLANT COORDINATOR I OR II, DEPENDING ON QUALIFICATIONS.

Transplant Coordinator I Requirements

Required Qualifications :

• Working knowledge of CMS / UNOS requirements
• Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
• Must have a minimum 1 year experience caring for transplant or outpatient dialysis patients within the last 3 years (ex. dedicated inpatient transplant unit, ambulatory transplant care, transplant coordinator role, outpatient dialysis center)
• Must demonstrate customer service skills appropriate to the job
• Minimum 2 years Critical Care, or Step Down, or Dialysis, or the Transplant setting
• Excellent written and verbal communication skills in English
• Demonstrated knowledge of immunosuppressive therapies and care of the post-transplant patient
• BSN Degree
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the organization
