Location : Victorville, California

Schedule : 5 days a week, 8-hour shifts

Job Description : An esteemed medical group is seeking a dedicated Advanced Practice Provider (NP or PA) for a prestigious Wellness Center located in Victorville. The successful candidate will be responsible for conducting medical annual wellness visits, focusing on hierarchical chronic condition codes. This role does not involve treatment; instead, it focuses on patient review, documentation, and ensuring compliance with Medicare guideline.

Key Responsibilities :

• Conducting medical annual wellness visits
• Utilizing hierarchical chronic conditions codes for accurate documentation
• Reviewing patient history thoroughly and documenting findings
• Facilitating referrals to primary care physicians as needed
• Ensuring compliance with Medicare guidelines for optimal reimbursement
• Managing a patient load of approximately 12 patients per day in 30-minute slots
