What You'll Do

• Collaborate with a multidisciplinary team to manage patient populations with chronic health conditions.
• Assess patients and develop individualized care plans for the care coordination program.
• Implement and evaluate care plans to ensure optimal health outcomes.
• Educate patients on disease processes, medications, and self-care strategies.
• Support patients through transitions of care to improve outcomes.
