Social Worker - Home Health

We are seeking a compassionate and dedicated Home Health Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and home health care, with experience supporting patients in an in-home or community-based setting.

In this role, you will provide a comprehensive range of social work services to home health patients and their families, including :

Conducting initial psychosocial, emotional, and environmental assessments

Providing ongoing counseling, education, and support to patients and caregivers

Assisting with community resources, referrals, and discharge planning

Collaborating with the clinical team to promote patient independence and well-being

As an essential member of the Interdisciplinary Team (IDT) , the Home Health Social Worker plays a vital role in assessing patient needs, developing individualized care plans, and ensuring services align with home health standards and regulatory requirements.

This position requires travel throughout the state of Mississippi coverage areas.
