What You'll Do

Drive consistent hospice and home health admissions growth

Build and manage referral partnerships across hospitals and senior-care settings

Host education sessions for families and providers to build trust

Hand off referrals fast—no bottlenecks, no red tape

Track results in modern CRM / EHR tools (streamlined, not clunky)

Partner with clinical teams to ensure smooth, compassionate transitions

Must-Haves

1+ year in hospice or home health marketing

Active driver's license + insured, reliable vehicle

Clean driving record

Strong written and verbal communication skills

Comfort with Microsoft Office and EHR systems

Perks & Pay

$70K–$85K base

Health, dental, and vision coverage

PTO (paid time off)

Mileage reimbursement at $0.70 / mile

Bonus : $1,500 / month at goal + $150 per referral above

Schedule & Setup

Mon–Fri

Field-based in Naperville and western suburbs

Travel ≤1 overnight per quarter
