Oncology Clinical Pharmacist - Pharmacy - Full-Time

The Clinical Pharmacist in Oncology is responsible for optimizing medication therapy and ensuring safe, effective, and cost-conscious pharmaceutical care for oncology patients. This includes cancer therapy education, monitoring for treatment-related toxicities, managing supportive care, and addressing comorbid conditions. The pharmacist collaborates with healthcare providers to evaluate treatment appropriateness, monitor efficacy and safety, and enhance patient adherence. Pharmaceutical care services include assessing patient needs, incorporating age- and disease-specific characteristics into drug therapy planning, educating patients, adjusting therapy based on clinical response, and preventing or mitigating medication-related adverse events, in collaboration with ordering provider. Clinical Pharmacist Specialists also serve as departmental resources and act as liaisons across hospital departments, interdisciplinary teams, and external organizations.

Requirements :

• Pharm D degree with a residency of one year or years equivalent.
• PGY in Oncology (preferred).
• Current licensure from the Louisiana Board of Pharmacy.
• Board Certification in Oncology (BCOP) preferred. If not certified, employees must complete ASHP Basics of Oncology in Pharmacy Certificate within days and BCOP within years of hire.
