Licensed Social Worker - Behavioral Health

DUTIES & RESPONSIBILITIES :

• Assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services programs within the facility in accordance with current existing Federal, State and local standards
• Develop and manage the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients
• Keep abreast of current federal and state regulations as well as professional standards
