Wisconsin Licensed Community Health Worker

A company is looking for a Community Health Worker - Remote in Wisconsin.

Key Responsibilities :

Engage members over the phone to discuss their health and create positive relationships

Support members in managing their health and accessing community resources

Conduct outreach to encourage participation in health-related programs and services

Required Qualifications :

High School Diploma / GED or higher

1+ years of clinical or case management experience

Intermediate experience with Microsoft Office (Word, Excel, Outlook)

Must reside in Wisconsin and have a designated quiet workspace

Availability to work between 8 : 00 am - 5 : 00 pm CST
