job_description.job_card.job_descriptionAttend a paid training program Meet patients and conduct a needs assessment Assist patients with getting medical care, organizing their records, making follow-up appointments, and filling prescriptions Help patients with social and basic needs like food, utilities, and rent assistance as well as building a social support network Help patients fill out applications for benefits like insurance and food stamps Make weekly follow-up calls to patients Motivate patients to meet their goals Help maintain an up-to-date directory of community resources Document work with patients served Prepare reports and documents as needed or requested Attend regular team meetings Travel required Bilingual in English / Spanish preferred Other duties as reasonably assigned Must already have a Community Health Worker Certificate - this is non-negotiable. HS Diploma or Ged Requires CHW Certification Excellent oral and written communication skills Fluency (verbal and written) in English; Bilingual in English / Spanish preferred Experience in outreach, customer service, social service, or a related field Ability to carry out written and oral instructions Ability to exercise judgment in the application of professional services Comfortable with home visits and outreach Comfortable with computer data entry Able and willing to work full time
