RN Service Coordinator Job Details :

Schedule : Monday - Friday, 8 : 00am - 5 : 00pm

Location : Field visits. (45 South area, Webster, Friendswood, League City, Galveston, Clear Lake, Pearland, Midtown, Downtown)

Temporary to hire

Starting Pay Rate : $40.38 / hour + (based on experience)

RN Service Coordinator Responsibilities :

• Perform telephonic and face-to-face clinical assessments to identify and manage members' physical, behavioral, and social service needs.
• Obtain member history to develop and implement individualized care plans in collaboration with members, caregivers, and providers.
• Identify high-risk members and coordinate care with the healthcare team to reduce complications and improve outcomes.
• Supervise and direct LPN / LVN, LSW, LSCW, LMSW, and non-RN clinicians in accordance with state laws and organizational policies.
• Interface with Medical Directors, Physician Advisors, and interdisciplinary teams to review and adjust care management treatment plans.
• Facilitate authorizations and referrals for services, ensuring compliance with benefit structures and regulatory standards.
• Provide mentorship and training to new staff and contribute to process improvement initiatives.
• Participate in quality assurance activities, audits, and internal reporting processes.
• Travel locally to member homes and community settings as needed to perform assessments.
